I/We affirm that my family and child/ren are registered and participate in the worship life of the faith community and participate regularly in the liturgy at Parish.
PLEDGE OF FINANCIAL COMMITMENT
I/We understand that significant decisions relating to financial management of St. Joseph School will be made in reliance upon my/our commitment in this agreement. I/We further understand that this agreement cannot be terminated except by mutual understandin g of St Joseph School and me/us.

PAYMENT AGREEMENT
1. For NEW FAMILIES, a one time, non-refundable enrollment fee of $55.00 per family is due upon registration.
2. I will sign the FACTS agreement regarding the payment of tuition.
3. I will pay the cost of tuition for the education of our child/ren at the St. Joseph School during the 2011 / 2012 school year in the sum of $ . Please put the full amount of tuition due and any scholarships will be deducted and the monthly payment adjusted accordingly.
4. I/We will pay a textbook fee of $ ($130.00 per student) payable by May 31, 2011.
5. I/We will pay a class fee of $ ($50.00 per student) payable by the June 15, 2011
If at anytime I/We are having a problem meeting our payment schedule, I/We will meet with the principal for any possible adjustments to this pay schedule.
¨ We will not be attending St. Joseph School in the 2011-2012 schoolyear.
SCHOOL ENHANCEMENT HOURS AGREEMENT
EXCELLENT SCHOOLS DON’T JUST HAPPEN - - - - - THEY’RE CREATED
At St. Joseph School, we believe that family involvement in a child’s education is a major factor in determining success in school and life. Studies have documented that family involvement contributes significantly to many positive outcomes, such as better school attendance, higher test scores and grades, improved homework completion rates, decreased violence and substance abuse, and higher graduation rates. In addition, we believe that volunteerism, charity, and teamwork are important social values that our children should see demonstrated by their parents, teachers, and parishioners. It is the combination of these values plus Christ-centered curriculum and academic excellence that will produce the leadership of tomorrow.
With this in mind, there is an expectation that each family will devote a minimum of FORTY (40) hours of SCHOOL ENHANCEMENT TIME to St. Joseph School during the 2011-12 academic year. A minimum of FIFTEEN(15) hours must be devoted to one or both of our two major fund raising activities, S.U.C.C.E.S.S. and the Auction. (Family participation in these activities is critical to help fill the gap between tuition and the actual cost of educating each child, $4375.) The remaining TWENTY-FIVE (25) hours will be devoted to those fundraisers and/or other activities the school has listed. Families who do not fulfill their volunteer obligation will be charged $20.00 per hour for each hour not served and $40 per hour not served for S.U.C.C.E.S.S. and/or the Auction of the 15 required hours. Each family is responsible to either pay $100 for procurements by December 1, 2011 or commit to contributing an item with a minimum $100 fair market value no later than January 30, 2012 for the annual auction. Each family not meeting the December 1st or January 30th cut off will be assessed an additional $25 buy out fee.
The attached form indicates a number of S.U.C.C.E.S.S. and Auction volunteer opportunities. Please indicate with a check mark the areas in which you wish to assist in order to meet your volunteer obligations. You will receive additional information regarding other volunteer opportunities throughout the school year. These options will range from things that can be done at the school during the day, evenings, and weekends and summer. There is something for everyone and we look forward to your contribution of time and talent.
